APPLICATION FORM FOR ADULT BAPTISM

(Applicable for candidates 16 years of age and above only)

Prerequisites for BAPTISM:

1. Worshipping in TPMC in-person Services regularly for at least 6 months.

2. Completed the ALPHA Course in TPMC or from another church (within the last 2 years by the date of the Baptism Service)

Documents to Submit for Baptism:

1. Submit a copy of your NRIC (Singaporean/PR) or FIN (Foreigner).
2. Submit a passport sized photo of yourself.
3. If you completed The Alpha Course in another church in the past 2 years, please submit a letter/email from the church confirming you completed the course.
