APPLICATION FORM FOR INFANT/CHILD BAPTISM

(Applicable for candidates from 1 month to 15 years of age)

Prerequisites for BAPTISM:

1. Either the parent(s) of the child or a sponsor (need not be related) must be a TPMC member.
2. Attend a briefing session for parents (details to be given).

Documents to Submit for Baptism:

1. Submit a copy of the child's Birth Certificate.
2. Submit a digital photo of the child.
